APPLICANT REQUESTS THAT:
The home of record on his DD Form 4/1, Enlistment/Reenlistment Document
Armed Forces of the United States, be changed from Eugene Oregon to Gold
Beach, Oregon.
APPLICANT CONTENDS THAT:
When he applied for the delayed entry program, his recruiter urged him
to write an address in the local area. He informed his recruiter he
currently lived with his parents in Gold Beach, Oregon and no longer
lived at a local address but he could supply an old address if
necessary. He was unaware the address would affect his Traffic
Management Office (TMO), shipment upon his separation.
In support of his request, the applicant submits copies of his DD Form
4/1 and SGLV 8286, Servicemembers Group Life Insurance Election and
Certificate.

STATEMENT OF FACTS:
The applicant initially entered the Regular Air Force on
5 February 2008.
On 4 October 2013, the applicant was furnished an honorable discharge,
and was credited with 5 years and 8 months of active service.
The applicants DD Form 4/1 indicates he accomplished a delayed entry in
the Air Force Reserve on 10 September 2007, at the Portland, Oregon
Military Entrance Processing Station (MEPS), and reflects his home of
record as an address in Eugene, Oregon.

responsibility (OPR), which is attached at Exhibit C.
AIR FORCE EVALUATION:
AFPC/DPSIPE recommends denial indicating there is no evidence of an
error or an injustice. DPSIPE states after a thorough review of the
military personnel record and other supporting documentation, there does
not appear to be an error or injustice in determining the applicants
home of record, which justifies a change. The DD Form 4/1 is the source
document for Home of Record (HOR) and Place of Entry (POE). The Joint
Federal Travel Regulation (JFTR), volume 1, appendix A 1, part 1,
states, The place recorded as the home of the individual when
reinstated, reappointed, or reenlisted remains the same as that recorded
when commissioned, appointed, enlisted or inducted, or ordered into the
tour of active duty, unless there is a break in service of more than one
full day. Only if a break in service exceeds one full day, may the
member change the HOR. The HOR is the location identified by the
individual upon entering the service, not a location where the
individual is later assigned or a location selected for convenience; a
correction must be fully justified. Domicile or legal residence may
change, but the HOR will not.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the applicant on 2
May 2014, for review and comment within 30 days (Exhibit D). To date, a
response has not been received.
THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing law or
regulations.
2. The application was timely filed.
3. Insufficient relevant evidence has been presented to demonstrate the
existence of an error or injustice. We took notice of the applicants
complete submission in judging the merits of the case; however, we agree
with the opinion and recommendation of the Air Force office of primary
responsibility (OPR) and adopt its rationale as the basis for our
conclusion the applicant has not been the victim of an error of
injustice. Therefore, in the absence of evidence to the contrary, we
find no basis to recommend granting the requested relief.
THE BOARD DETERMINES THAT:
The applicant be notified the evidence presented did not demonstrate the
existence of material error or injustice; the application was denied
without a personal appearance; and the application will only be
reconsidered upon the submission of newly discovered relevant evidence
not considered with this application.
